What we measure, why we measure it, and how we use data to keep getting better for the young people in our care.

Evidence-based care

Outcomes matter — here is what we track

Local authorities are accountable for the outcomes of their care leavers. So are we. Every placement at C4U Homes is monitored against a structured framework — not because regulation demands it, but because we believe young people deserve better than vague promises.

Our Framework

The seven domains we monitor

Every young person has an Outcomes Plan reviewed monthly. We track measurable progress across these seven areas:

Education, Employment & Training (EET)

We support engagement in college, apprenticeships, traineeships, and employment.

Metrics we track: % of residents in EET, attendance rates, qualifications gained, employment hours per week

Placement Stability

Stability is the foundation of every other outcome. Frequent moves harm young people.

Metrics we track: Average length of stay, planned vs unplanned endings, emergency moves, return-to-base rate after missing episodes

Emotional Wellbeing

Trauma-informed support, with formal links to CAMHS, GP and counselling.

Metrics we track: Goal-Based Outcomes (GBO) scoring, mental health appointments attended, self-reported wellbeing surveys, crisis incidents

Life Skills & Independence

Progress towards independent living — measured, not assumed.

Metrics we track: Independence Skills Assessment scores (baseline vs review), cooking competency, budgeting, tenancy readiness scoring

Safeguarding & Safety

Reducing risks; documenting and learning from every incident.

Metrics we track: Missing episodes (frequency & duration), contextual safeguarding risk reduction, police involvement, return interview completion within 72 hours

Engagement & Voice

Young people drive their own care plans. Engagement is non-negotiable.

Metrics we track: Key-working session attendance, plan review participation, satisfaction surveys (quarterly), formal feedback recorded
🚀

Successful Move-On

The ultimate measure: did the young person leave us ready for the next stage of life?

Metrics we track: % moving to independent accommodation, sustained tenancies at 6 and 12 months post-leaving, return-to-care rate, ongoing EET status
Reporting

How we share outcomes data

Transparency is core to our practice. Commissioners and placing authorities can expect:

  • Monthly placement reports — progress summary against each domain, sent to the allocated social worker
  • Quarterly outcomes review — structured review of all measurable indicators
  • Annual provider report — aggregate outcomes data published to commissioners
  • Ad-hoc updates — any significant change in risk, achievement or status reported within 24 hours
  • End-of-placement summary — comprehensive document handed over to support move-on planning

Publishing our outcomes

Once our first cohort of young people has been placed, we will publish anonymised aggregate outcomes data quarterly on this page. Commissioners are welcome to request our outcomes framework in full at any time.
